We know that from time to time, you may need to return a product purchased from QCTV. We try to make the process go as smoothly as possible. For that reason, we provide this online form to initiate a Return Merchandise Authorization (or RMA).

Please complete the form below and click the submit button. Submit one RMA for each item you are returning. If you do not know the answer to one of the questions enter "unknown." All * fields are required. A Customer Service Representative will review the form and contact you if necessary. If you have problems submitting this form you may print out a hard copy and fax it to us. Once we receive your information you will then receive an RMA number and return instructions by email. -- Thank you.

Please note: All returns for credit are subject to a 15% restocking fee and are allowed only within 30 days from the date of purchase. Whenever returning an item to QCTV, we require the following packing and shipping methods: Lamp hours cannot exceed 4 hours of usage. Please return all products in their original packaging with original documentation. Clearly place the RMA # on a label and attach it to the outside of the box. Please do not write the RMA # directly on the box. Include the RMA # in the "Ship To" address line. Otherwise, customer pays shipping costs and must insure the package for the purchased value. Acceptable shipping carriers are UPS ONLY. Do not send return merchandise via U.S. Mail by any method; products returned by U.S. Mail will not be credited or exchanged.
